Case 22 - 22 year old male with sudden knee pain while playing soccer
22 year old male with left knee injury while playing soccer.
- Most of the elements of this case should be familiar: a moderate joint effusion, kissing contusions of the posterolateral tibial plateau and posterolateral femoral condyle, and a complete tear of the ACL.
- This pattern of injuries has a high association with meniscal injury. Remember, the body of the menisci are best seen on coronal views, while the anterior and posterior horns are best seen on sagittal views.
- On closer inspection we see focal tears of both the medial and lateral menisci: a vertical band of fluid signal through the peripheral posterior horn of the medial meniscus and a small radial band of intermediate signal through the posterior root of the lateral meniscus.
- In summary, this patient has multiple acute injuries including an ACL tear with kissing contusions and tears of the medial and lateral menisci.
