Case 12 - 18 year old female with a soccer injury 10 months prior
18 year old female with left knee pain following a soccer injury 10 months prior.
- Any MR evaluation of the knee must include an assessment of the stabilizing ligaments, including the anterior and posterior cruciate ligaments of the knee.
- This case is a bit challenging because the injury occurred 10 months ago. Often with acute injuries to the anterior cruciate ligament (ACL) there is a large amount of fluid and hemorrhage at the site of the ACL; however, here the primary finding is simply that the ACL is not well visualized.
- Even on axial and coronal images, the normal T2 hypointense bundles of the ACL in the intercondylar notch are simply not seen, indicating a chronic ACL tear.
- There's one other important finding. Did you see it? There's a focal full-thickness defect in the lateral femoral condyle cartilage at the intercondylar notch with an adjacent intra-articular body.
