Case 13 - 48 year old female with knee pain
48 year old female with chronic right knee pain.
- Unlike the last case, we can clearly identify the anterior cruciate ligament within the intercondylar notch; however, the ligament appears thickened with high T2 signal throughout the central ligament.
- How can we distinguish this from a partial ACL tear? First, the surrounding T2 hypointense fibers appear intact. Second, there are no secondary signs of an ACL injury, such as a joint effusion or osseous contusion.
- This appearance is compatible with mucoid degeneration of the ACL.
- This patient also has abnormal signal and superficial fraying of the articular cartilage, particularly within the patellofemoral compartment.
