Case 45 - 30 year old female with right knee pain following ACL reconstruction
30 year old female with medial right knee pain; prior ACL reconstruction.
- This case shouldn't be too challenging, as it reiterates several of the findings we've previously discussed.
- On the coronal PD images we again notice an extra structure within the intercondylar notch. Additionally the body of the medial meniscus appears truncated and small.
- On the sagittal PD images we have another excellent example of a double PCL sign.
- The axial images clinch the diagnosis of a flipped bucket handle tear of the medial meniscus.
- Despite the susceptibility artifact from the interference screws, we can confidently state that the ACL graft appears intact.
- Did you notice this defect in the central third of the patella? This is the site of the graft harvest for this particular ACL reconstruction.
- Did you notice this large multiloculated fluid collection positioned between the vastus medialis muscle and the adductor magnus tendon? We can generically refer to such fluid collections as a ganglion. If these collections are symptomatic they may be drained under ultrasound guidance. In this particular instance you can actually follow this fluid signal inferiorly until is actually communicates with a small parameniscal cyst adjacent to the bucket handle tear. This ganglion may simply be a large, atypical parameniscal cyst.
