- Do you see the pattern emerging? The coronal and sagittal proton density images are the most helpful in an initial inspection of the menisci. This patient has a small free edge tear of the posterior horn of the medial meniscus. This vertically oriented tear has both radial and longitudinal components, best seen on the axial proton density images. Even though the tear is small and non displaced, we now know that this oblique orientation indicates a flap or parrot beak meniscal tear.
- This patient also has some subtle focal edema in the superolateral Hoffa's fat pad. This raises the possibility of an impingement syndrome.
Always account for the whole volume of the menisci. A chronically displaced tear may simply appear as a diminutive or truncated meniscus.
