Case 6: 22 year old male basketball player with left hip pain.
- A small ossicle adjacent to the superior acetabulum laterally is compatible with a small os acetabulum. Note also that there is suggestion of prominence of the bony head-neck junction of the left femur as well.
- These findings are somewhat better appreciated on the plain film, are bilateral, and can be seen in the setting of cam-type femoral acetabular impingement (FAI). FAI can be categorized as cam-type, where the femoral head-neck junction is abnormal, or pincer-type, where the acetabulum itself is abnormal.
- Cam-type FAI is often associated with injury to the cartilage, manifested here as degenerative tearing of the anterior superior labrum. Note the loss of normal triangular morphology and solid black appearance of the labrum.
- The articular cartilage is also at risk, as seen here with a focal defect in the superior aspect of the cartilage.
