Case 20 - left hip pain
Case 20: Diffuse left hip pain lasting greater than six months, exacerbated with running and walking.
- There are innumerable, tiny, intermediate intensity filling defects distributed throughout the joint fluid of the left hip. Findings are compatible with EXTENSIVE REACTIVE SYNOVITIS, although other causes of synovial proliferation are in the differential, including synovial chondromatosis, inflammatory arthropathy, or pigmented villonodular synovitis (PVNS).
- Note that the bone marrow signal is preserved without evidence of erosions, which can be seen in entities like synovial chondromatosis or PVNS.
- There is a small linear focus of increased signal suggesting extension of the gadolinium contrast from the joint space, compatible with a small focal anterior labral tear.
