Tear of the distal attachment of the superomedial bundle, with thickening of the proximal fibers. Findings are consistent with chronic sprain/partial tear.The medioplantar oblique bundle is very diminutive, which may be congenital or due to a chronic tear.
The inferoplantar longitudinal bundle is not well seen.
Of note, there is a chronic split tear (left) of the posterior tibial tendon, and an accessory navicular bone (right).
