Case 08 - 34 year old male with knee pain
34 year old male with acute onset left knee pain and catching while running.
- The patient has a sizable joint effusion.
- There are multiple areas of injury to the articular cartilage, most severe in the patellofemoral joint. Note the patellofemoral compartment is best evaluated on axial and sagittal views.
- Note the multiple areas of signal abnormality in the patellar cartilage with diffuse supericial fraying. Similar focal areas of signal abnormality are also seen in the articular cartilage overlying the medial trochlea and medial femoral condyle.
- A deeper fissure is seen articular cartilage of the lateral trochlea. More importantly there is a large area of adjacent delamination. This can be seen as a linear band of fluid signal undercutting intact articular cartilage. It's important for the surgeon's to note the size of the delamination -- approximately 9 x 12 mm in this case.
- The history and presence of a joint effusion suggest an acute cartilage injury with a large area of delamination.
