Case 16 - 36 year old female with knee pain
36 year old female with a recent "knee sprain."
- Does this pattern of findings look familiar? We again have a joint effusion with a fluid-fluid level indicating hemarthrosis from an acute injury.
- Interestingly, we see some blood products layering dependently in a fluid-collection posterior to the knee; this is a small Baker cyst. A Baker cyst is a fluid-distended bursa, which communicates with the joint; this communication is always positioned between the medial gastrocnemius and semimembranosus tendons.
- We again see focal contusion at the posterolateral tibial plateau.
- Finally, when we inspect the stabilizing ligaments, we see extensive edema in the intercondylar notch and complete discontinuity of the ACL fibers indicating an acute ACL tear.
