Case 48 - 18 year old male with anterior right knee pain

18 year old male hurdler with chronic anterior left knee pain.

Case discussion:
  • Jumper's knee: focal partial tear of proximal patella and mild fragmentation of the inferior patellar pole
  • This patient also has mild patella alta. This simply refers to the high positioning of the patella relative to the trochlea, which is often seen in association with other disorders of the extensor mechanism. On MRI patellar alta is diagnosed on the sagittal images if the ratio of the length of the patellar tendon to the patella is greater than 1.5.

 Expert commentary:
In adolescent patients when the degree of fragmentation of the inferior patellar pole is more severe, this constellation of findings is often referred to by the eponym Sinding-Larsen-Johansson syndrome. It's simply another form of chronic patellar tendinosis. When the fragmentation occurs at the patellar tendon insertion on the tibial tuberosity the disease is referred to by the eponym Osgood-Schlatter syndrome. Both disease entities respond well to conservative management.
