54 year old male with medial joint line pain in the left knee.
- Much like the last case we can see a vertically oriented tear in the posterior horn of the medial meniscus; however, on sagittal and axial images we can see the tear also has a longitudinal component. This type of obliquely oriented tear is referred to as a flap tear or parrot beak tear; one can imagine if the longitudinal component extends too far the torn meniscal flap can easily become displaced.
- There's two less important incidental findings as well: a small baker cyst and a bipartite patella. Remember that the bipartite patella always occurs at the upper outer pole.
Flap or parrot-beak configuration meniscal tears most commonly occur in the posterior horn of the medial meniscus. They frequently present with locking or catching of the joint due to the meniscal flap.
