- This patient has a large joint effusion with synovitis.
- The primary finding in this case is subtle compared to the previous case: there's a 6-mm radial tear in the posterior horn of the medial meniscus.
Radial tears are vertical meniscal tears oriented perpendicular to the long axis of the meniscus. They frequently occur as a result of acute axial loading, such as in a motor vehicle collision. They're often best seen on coronal and axial proton density images. Tears near the posterior root of the medial meniscus, such as this one, are especially important to identify as they can lead to accelerated osteoarthritis if left untreated.
