42 year old male with sudden onset anterior knee pain while jumping three days prior.

Case discussion:
  • Complete patellar tendon tear with 1-cm retraction; superimposed on chronic patellar tendinopathy
  • ? biceps femoris tendinopathy
  • Moderate degenerative changes with areas of full thickness cartilage loss in the patellofemoral compartment

Expert commentary:
High-grade tears of the patellar tendon require a large amount of axial force on a flexed knee and often occur in the setting of underlying tendinopathy. Complete and near-complete tears of the patellar tendon, such as this one, are often considered an orthopedic surgical emergency. It's important to describe the location and extent of the tear for the surgeons preoperatively, as well as the degree of underlying tendinopathy and tendon retraction.
