The superomedial bundle has intermediate signal consistent with old sprain or degeneration.The medioplantar oblique and the inferoplantar longitudinal bundles appear intact.
Tendinosis and partial tearing of the posterior tibial tendon with tenosynovitis. Of note, a lobular soft tissue mass abutting the PT tendon is present, which may represent focal synovitis vs ganglion cyst.
