Case 11

Avulsion fracture of the fibular head ("arcuate sign"). The biceps femoris remains attached this fracture fragment, but appears split. The fibular collateral ligament appears markedly lax and indistinct, consistent with high grade vs complete tearing. The iliotibial band is macerated and torn.

High grade tearing of the popliteus tendon and myotendinous junction. Similarly, the popliteofibular ligament is torn with the free torn edge easily visible posterior to the fibular head.

Of note, the common peroneal nerve appears enlarged and hyperintense with surrounding fluid signal, consistent with traumatic traction injury.
